Goal Setting Worksheet

Having goals is an important part of this process.  It gives you something to strive for, something to keep you motivated, and something to CELEBRATE!

Before you start off any new program to improve your health, take a few minutes to complete this worksheet.

(Be as specific as possible. How many pounds will you lose? What size clothes do you want to wear? What health medications do you want to hopefully get off from?):

(What are your trigger situations? List some things that have derailed your efforts in the past):
